Your First Visit

We look forward to meeting you on your first visit to PT Pros! In order to create an individualized treatment program, your Physical Therapist (PT) will perform a thorough evaluation on your first visit. This simply begins with your PT asking you to describe the problem that requires physical therapy, as well as any other medical issues you may have. Your PT will then perform a comprehensive physical examination in order to assess the problem. By the end of your first visit, you will have a complete understanding of your evaluation and treatment program. We understand that each patient’s physical therapy treatment is different, and we promise to give you the personal attention you deserve.

After we have completed your evaluation, your physical therapist will explain all aspects of your physical therapy treatment in detail. We will provide a complete clinical diagnosis regarding the nature of the problem, a written summary of the findings and a detailed plan that outlines your treatment, goals and estimated fees. We encourage questions from our patients and as such, we provide ample time to answer your questions.

What Should I Bring?

Arrive at least 15 minutes early in order to complete any remaining paperwork. Wear loose-fitting, comfortable clothing and shoes. Your physical therapist will want to view the area of your body that he/she will be treating, so shorts or a sports bra might be helpful.

Please bring with you:

  • Current copy of insurance card(s)
  • Photo ID
  • Doctor’s referral (if applicable)
  • Complete list of any prescription or over-the-counter medicines and/or supplements that you are currently taking
  • Results from MRI, X-ray, or other diagnostic tests (if applicable)
  • Attorney contact information (if applicable)
  • Allow for 1 to 1 ½ hours to complete the initial evaluation

We promise to speak candidly with you about our diagnosis and recommendations, any alternative treatments and any symptoms present that may require further treatment. Working together, we can achieve a true partnership, with one common goal – helping you get back to doing the things you love, pain-free!
